<p style="-uslm-lc:I11" class="indent0">Whoever falsely makes, forges, or counterfeits in whole or in part, any form or request in similitude of the form or request provided by the Government for requesting a common carrier to furnish transportation on account of the United States or any department or agency thereof, or knowingly alters any form or request provided by the Government for requesting a common carrier to furnish transportation on account of the United States or any department or agency thereof; or</p>
<p style="-uslm-lc:I11" class="indent0">Whoever knowingly passes, utters, publishes, or sells, or attempts to pass, utter, publish, or sell, any such false, forged, counterfeited, or altered form or request—</p>
<p style="-uslm-lc:I11" class="indent0">Shall be fined under this title or imprisoned not more than ten years, or both.</p>
