Such annual reports shall include—</chapeau><paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0c188-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/a/1\"><num value=\"1\">(1)</num><content> statistical data reflecting services and activities provided to individuals during the preceding fiscal year;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0c189-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/a/2\"><num value=\"2\">(2)</num><content> statistical data collected under <ref href=\"/us/usc/t42/s3012/a/16\">section 3012(a)(16) of this title</ref>;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0c18a-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/a/3\"><num value=\"3\">(3)</num><content> statistical data and an analysis of information regarding the effectiveness of the State agency and area agencies on aging in targeting services to older individuals with greatest economic need and older individuals with greatest social need, with particular attention to low-income minority individuals, older individuals residing in rural areas, low-income individuals, and frail individuals (including individuals with any physical or mental functional impairment); and</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0c18b-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/a/4\"><num value=\"4\">(4)</num><content> a description of the implementation of the plan required by <ref href=\"/us/usc/t42/s3012/a/14\">section 3012(a)(14) of this title</ref>.</content>\n</paragraph>\n</subsection>\n<subsection style=\"-uslm-lc:I19\" class=\"indent2 firstIndent-2\" id=\"idaab0c18c-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b\"><num value=\"b\" class=\"bold\">(b)</num><heading class=\"bold\"> Report on ombudsman program</heading><paragraph style=\"-uslm-lc:I11\" class=\"indent0\" id=\"idaab0c18d-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/1\"><num value=\"1\">(1)</num><chapeau> Not later than March 1 of each year, the Assistant Secretary shall compile a report—</chapeau><subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0c18e-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/1/A\"><num value=\"A\">(A)</num><content> summarizing and analyzing the data collected under subchapters III and XI in accordance with <ref href=\"/us/usc/t42/s3058g/c\">section 3058g(c) of this title</ref> for the then most recently concluded fiscal year;</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e89f-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/1/B\"><num value=\"B\">(B)</num><content> identifying significant problems and issues revealed by such data (with special emphasis on problems relating to quality of care and residents’ rights);</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a0-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/1/C\"><num value=\"C\">(C)</num><content> discussing current issues concerning the long-term care ombudsman programs of the States; and</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a1-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/1/D\"><num value=\"D\">(D)</num><content> making recommendations regarding legislation and administrative actions to resolve such problems.</content>\n</subparagraph>\n</paragraph>\n<paragraph style=\"-uslm-lc:I11\" class=\"indent0\" id=\"idaab0e8a2-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/2\"><num value=\"2\">(2)</num><chapeau> The Assistant Secretary shall submit the report required by paragraph (1) to—</chapeau><subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a3-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/2/A\"><num value=\"A\">(A)</num><content> the Special Committee on Aging of the Senate;</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a4-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/2/B\"><num value=\"B\">(B)</num><content> the Committee on Education and Labor of the House of Representatives; and</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a5-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/2/C\"><num value=\"C\">(C)</num><content> the Committee on Health, Education, Labor, and Pensions of the Senate.</content>\n</subparagraph>\n</paragraph>\n<paragraph style=\"-uslm-lc:I11\" class=\"indent0\" id=\"idaab0e8a6-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/3\"><num value=\"3\">(3)</num><chapeau> The Assistant Secretary shall provide the report required by paragraph (1), and make the State reports required under subchapters III and XI in accordance with <ref href=\"/us/usc/t42/s3058g/h/1\">section 3058g(h)(1) of this title</ref> available, to—</chapeau><subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a7-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/3/A\"><num value=\"A\">(A)</num><content> the Administrator of the Centers for Medicare &amp; Medicaid Services;</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a8-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/3/B\"><num value=\"B\">(B)</num><content> the Office of the Inspector General of the Department of Health and Human Services;</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8a9-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/3/C\"><num value=\"C\">(C)</num><content> the Office of Civil Rights of the Department of Health and Human Services;</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8aa-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/3/D\"><num value=\"D\">(D)</num><content> the Secretary of Veterans Affairs; and</content>\n</subparagraph>\n<subparagraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8ab-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/b/3/E\"><num value=\"E\">(E)</num><content> each public agency or private organization designated as an Office of the State Long-Term Care Ombudsman under subchapter III or XI in accordance with <ref href=\"/us/usc/t42/s3058g/a/4/A\">section 3058g(a)(4)(A) of this title</ref>.</content>\n</subparagraph>\n</paragraph>\n</subsection>\n<subsection style=\"-uslm-lc:I19\" class=\"indent2 firstIndent-2\" id=\"idaab0e8ac-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/c\"><num value=\"c\" class=\"bold\">(c)</num><heading class=\"bold\"> Outreach activities; report on evaluations to be included in annual report</heading><chapeau style=\"-uslm-lc:I11\" class=\"indent0\">The Assistant Secretary shall, as part of the annual report submitted under subsection (a), prepare and submit a report on the outreach activities supported under this chapter, together with such recommendations as the Assistant Secretary deems appropriate. In carrying out this subsection, the Assistant Secretary shall consider—</chapeau><paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8ad-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/c/1\"><num value=\"1\">(1)</num><content> the number of older individuals reached through the activities;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8ae-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/c/2\"><num value=\"2\">(2)</num><content> the dollar amount of the assistance and benefits received by older individuals as a result of such activities;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8af-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/c/3\"><num value=\"3\">(3)</num><content> the cost of such activities in terms of the number of individuals reached and the dollar amount described in paragraph (2);</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8b0-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/c/4\"><num value=\"4\">(4)</num><content> the effect of such activities on supportive services and nutrition services furnished under subchapter III of this chapter; and</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8b1-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/c/5\"><num value=\"5\">(5)</num><content> the effectiveness of State and local efforts to target older individuals with greatest economic need (including low-income minority individuals and older individuals residing in rural areas) and older individuals with greatest social need (including low-income minority individuals and older individuals residing in rural areas) to receive services under this chapter.</content>\n</paragraph>\n</subsection>\n<subsection style=\"-uslm-lc:I19\" class=\"indent2 firstIndent-2\" id=\"idaab0e8b2-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/d\"><num value=\"d\" class=\"bold\">(d)</num><heading class=\"bold\"> Evaluation to Congress</heading><chapeau style=\"-uslm-lc:I11\" class=\"indent0\">The Assistant Secretary shall provide the evaluation required under <ref href=\"/us/usc/t42/s3017/b\">section 3017(b) of this title</ref> to—</chapeau><paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8b3-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/d/1\"><num value=\"1\">(1)</num><content> the Committee on Health, Education, Labor, and Pensions of the Senate;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8b4-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/d/2\"><num value=\"2\">(2)</num><content> the Committee on Appropriations of the Senate;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8b5-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/d/3\"><num value=\"3\">(3)</num><content> the Special Committee on Aging of the Senate;</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab0e8b6-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/d/4\"><num value=\"4\">(4)</num><content> the Committee on Education and Labor of the House of Representatives; and</content>\n</paragraph>\n<paragraph style=\"-uslm-lc:I12\" class=\"indent1\" id=\"idaab10fc7-09a0-11eb-a85b-f5cef3d06f4d\" identifier=\"/us/usc/t42/s3018/d/5\"><num value=\"5\">(5)</num><content> the Committee on Appropriations of the House of Representatives.</content>\n</paragraph>\n</subsection>\n<sourceCredit id=\"idaab10fc8-09a0-11eb-a85b-f5cef3d06f4d\">(<ref href=\"/us/pl/89/73/tII/s207\">Pub.
