<section xmlns="http://xml.house.gov/schemas/uslm/1.0" xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" xmlns:dc="http://purl.org/dc/elements/1.1/" xmlns:dcterms="http://purl.org/dc/terms/" style="-uslm-lc:I80" id="idaac22736-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c"><num value="3032c">§ 3032c.</num><heading> Health care service demonstration projects in rural areas</heading><subsection style="-uslm-lc:I19" class="indent2 firstIndent-2" id="idaac22737-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/a"><num value="a" class="bold">(a)</num><heading class="bold"> Authority</heading><content><p style="-uslm-lc:I11" class="indent0">The Assistant Secretary, after consultation with the State agency of the State involved, shall make grants to eligible public agencies and nonprofit private organizations to pay part or all of the cost of developing or operating model health care service projects (including related home health care services, adult day health care, mental health services, outreach, and transportation) through multipurpose senior centers that are located in rural areas and that provide nutrition services under <ref href="/us/usc/t42/s3030e">section 3030e of this title</ref>, to meet the health care needs of medically underserved older individuals residing in such areas.</p>

</subsection>
<subsection style="-uslm-lc:I19" class="indent2 firstIndent-2" id="idaac22738-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b"><num value="b" class="bold">(b)</num><heading class="bold"> Eligibility</heading><chapeau style="-uslm-lc:I11" class="indent0">To be eligible to receive a grant under subsection (a), a public agency or nonprofit private organization shall submit to the Assistant Secretary an application containing such information and assurances as the Secretary may require, including—</chapeau><paragraph style="-uslm-lc:I12" class="indent1" id="idaac22739-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/1"><num value="1">(1)</num><chapeau> information describing the nature and extent of the applicant’s—</chapeau><subparagraph style="-uslm-lc:I13" class="indent2" id="idaac2273a-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/1/A"><num value="A">(A)</num><content> experience in providing medical services of the type to be provided in the project for which a grant is requested; and</content>
</subparagraph>
<subparagraph style="-uslm-lc:I13" class="indent2" id="idaac2273b-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/1/B"><num value="B">(B)</num><chapeau> coordination and cooperation with—</chapeau><clause style="-uslm-lc:I14" class="indent3" id="idaac2273c-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/1/B/i"><num value="i">(i)</num><content> institutions of higher education having graduate programs with capability in public health, mental health, the medical sciences, psychology, pharmacology, nursing, social work, health education, nutrition, or gerontology, for the purpose of designing and developing such project; and</content>
</clause>
<clause style="-uslm-lc:I14" class="indent3" id="idaac24e4d-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/1/B/ii"><num value="ii">(ii)</num><content> critical access hospitals (as defined in <ref href="/us/usc/t42/s1395x/mm/1">section 1395x(mm)(1) of this title</ref> and rural health clinics (as defined in <ref href="/us/usc/t42/s1395x/aa/2">section 1395x(aa)(2) of this title</ref>);</content>
</clause>
</subparagraph>
</paragraph>
<paragraph style="-uslm-lc:I12" class="indent1" id="idaac24e4e-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2"><num value="2">(2)</num><chapeau> assurances that the applicant will carry out the project for which a grant is requested, through a multipurpose senior center located—</chapeau><subparagraph style="-uslm-lc:I13" class="indent2" id="idaac24e4f-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2/A"><num value="A">(A)</num><clause style="-uslm-lc:I13" class="indent2" id="idaac24e50-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2/A/i"><num value="i">(i)</num><content> in a rural area that has a population of less than 5,000; or</content>
</clause>
<clause style="-uslm-lc:I13" class="indent2" id="idaac24e51-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2/A/ii"><num value="ii">(ii)</num><content> in a county that has fewer than seven individuals per square mile; and</content>
</clause>
</subparagraph>
<subparagraph style="-uslm-lc:I13" class="indent2" id="idaac24e52-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2/B"><num value="B">(B)</num><chapeau> in a State in which—</chapeau><clause style="-uslm-lc:I14" class="indent3" id="idaac24e53-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2/B/i"><num value="i">(i)</num><content> not less than 33⅓ of the population resides in rural areas; and</content>
</clause>
<clause style="-uslm-lc:I14" class="indent3" id="idaac24e54-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/2/B/ii"><num value="ii">(ii)</num><content> not less than 5 percent of the population resides in counties with fewer than seven individuals per square mile,</content>

<continuation style="-uslm-lc:I32" class="indent2 firstIndent0">as defined by and determined in accordance with the most recent data available from the Bureau of the Census; and</continuation>
</subparagraph>
</paragraph>
<paragraph style="-uslm-lc:I12" class="indent1" id="idaac24e55-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/b/3"><num value="3">(3)</num><content> assurances that the applicant will submit to the Assistant Secretary such evaluations and reports as the Assistant Secretary may require.</content>

<subsection style="-uslm-lc:I19" class="indent2 firstIndent-2" id="idaac24e56-09a0-11eb-a85b-f5cef3d06f4d" identifier="/us/usc/t42/s3032c/c"><num value="c" class="bold">(c)</num><heading class="bold"> Reports</heading><content><p style="-uslm-lc:I11" class="indent0">The Assistant Secretary shall prepare and submit to the appropriate committees of Congress a report that includes summaries of the evaluations and reports required under subsection (b).</p>
