<section xmlns="http://xml.house.gov/schemas/uslm/1.0" xmlns:xsi="http://www.w3.org/2001/XMLSchema-instance" xmlns:dc="http://purl.org/dc/elements/1.1/" xmlns:dcterms="http://purl.org/dc/terms/" style="-uslm-lc:I80" id="id51111614-f67c-11e5-87ee-a95dc0577289" identifier="/us/usc/t5/s8907"><num value="8907">§ 8907.</num><heading> Information to individuals eligible to enroll</heading><subsection style="-uslm-lc:I11" class="indent0" id="id51111615-f67c-11e5-87ee-a95dc0577289" identifier="/us/usc/t5/s8907/a"><num value="a">(a)</num><content> The Office of Personnel Management shall make available to each individual eligible to enroll in a health benefits plan under this chapter such information, in a form acceptable to the Office after consultation with the carrier, as may be necessary to enable the individual to exercise an informed choice among the types of plans described by sections 8903 and 8903a of this title.</content>
</subsection>
<subsection style="-uslm-lc:I11" class="indent0" id="id51111616-f67c-11e5-87ee-a95dc0577289" identifier="/us/usc/t5/s8907/b"><num value="b">(b)</num><chapeau> Each enrollee in a health benefits plan shall be issued an appropriate document setting forth or summarizing the—</chapeau><paragraph style="-uslm-lc:I12" class="indent1" id="id51111617-f67c-11e5-87ee-a95dc0577289" identifier="/us/usc/t5/s8907/b/1"><num value="1">(1)</num><content> services or benefits, including maximums, limitations, and exclusions, to which the enrollee or the enrollee and any eligible family members are entitled thereunder;</content>
</paragraph>
<paragraph style="-uslm-lc:I12" class="indent1" id="id51111618-f67c-11e5-87ee-a95dc0577289" identifier="/us/usc/t5/s8907/b/2"><num value="2">(2)</num><content> procedure for obtaining benefits; and</content>
</paragraph>
<paragraph style="-uslm-lc:I12" class="indent1" id="id51111619-f67c-11e5-87ee-a95dc0577289" identifier="/us/usc/t5/s8907/b/3"><num value="3">(3)</num><content> principal provisions of the plan affecting the enrollee and any eligible family members.</content>
